1. 請您本人攜帶換發之「最新身分證件(居留證)」正本，與原招攬人員聯絡辦理變更統一證號作業事宜。

2. 請您本人攜帶換發之「最新身分證件(居留證)」正本，親臨本公司辦理變更統一證號作業事宜。

3. 請您本人填寫「保險契約基本資料變更申請書_A 版」並檢附「最新身分證件(居留證)」影本，郵寄至本公司辦理變更統一證號作業事宜。

Notice for Foreign Nationals Applying to Update the“New UI No.”

Dear Customers, In response to the “Ministry of the Interior´s Policy of Promoting the Issuance of the New UI Number for the Foreign Nationals”, if you have already been issued a new UI Number, you are reminded of the following:

1.Please bring the original copy of your “latest identity certificate (Residence Permit)” and contact your Original service staff to change your UI Number.

2.Please bring the original copy of your “latest identity certificate (Residence Permit)” to First Life Insurance Co., Ltd. to change your UI Number.

3.Please fill in the linked Application Form for Change of Insurance Party Particulars - (A)" latest identity certificate (Residence Permit) attached ", and mail it to First Life Insurance Co., Ltd. to change your UI Number.
